meropenem not effective in enteric fever

i am seeing many pts who were given meropenem for enteric fever for 8 to 10 days by some pediatricians.these pts continued to have high fever.these all pts responded very well either to ceftriaxone alone or cxone+ofloxacin within 4 to5 days.one pt responded nicely to oral cefuroxime.using meropenem for enteric fever is in my opinion wastage of both time and money.

rickettial fever on the rise

we are seeing more and more cases of rickettial fever in children.we have managed 3 cases of it in last 15 days.all patients recovered and sent home on day 3 with adv to complete 10 days of abs course. 

epidemic of dengue and gastroenteritis.

we have managed at least 30 pts of dengue and dengue like illnesses in last 30 days.many r coming with dengue shock,and many with capillary leaks.the duration of stay is from 3 to8 days.many with shock and capillary leaks required platelet and ffp,while majority required only supportive care.there is sudden increase in no of acute gastroenterits pts inlast 7 days.

non surgical mangement of intussusception

About 10 children with intussusception were hospitalized in MCH in last 6 months.of these 8 were managed by hydrostatic reduction technique[ conservatively] & went home. only 2 required surgery.no patients who were conserved had recurrence ,while 1 operated pt had second intussusception.he was again managed conservatively.

one 18 months old male child presented with convulsions ,fever & unconciousness.inv revealed big midbrain sol and mild hydrocephalus.pt managed successfully &sent home.he is on AKT at present.he is fully concious playfull with minimal focal residual neurological defects.(details on request.)

after 2wks of treatment pt showed 90% improvement with mild monoparesis.

Two more academic cases managed successfully.

one pt came with h/o severe headache ,fever ,disorientation and ataxia.cliniclly suspected meningitis.cbc ,csf,ct brain revealed nothing.on day 2 he developed rt facial palsy.clinical diagnosis was ADEM(acute disseminated enceohalo myelitis).pt was started iv methylprednisolone and symptomatic t/t.he showed improvement after 2 days.he was discharged on day 6 with almost 80 % recovery.One pt came with h/o convulsions since 5 days.he was t/ted at one picu with no responce.this pt responded to our management in 2 days and is seizer free since last 4 days.

Today i delivered a lecture on hep b at a camp organised by jamat e islami hind.it was a gen knowledge lect.it was followed by short q & a session.it was in urdu &was appreciated by all.
